Our story
Our story begins with a family in medicine.
Family Health Medical Centre was established in Ang Mo Kio in 1993 by Dr Vida Chou. For more than three decades it has cared for patients and families at different stages of their lives.
Alongside it is the neurosurgical practice of Dr Chou Ning, which treats conditions of the brain and spine. Together, they are the family foundation behind Continuum: community-based medical care and specialist expertise.
Founded by Dr Walter Chou, Continuum Health Group brings these foundations together so patients can reach medical care more easily and keep receiving support as their needs change.
Continuum 24/7 Clinic in Sengkang offers family medicine around the clock. It is a community practice for acute illnesses, ongoing medical needs, preventive health and the care of families.
As we grow, we want to keep the personal responsibility of medical practice while building a more connected healthcare network.
Why Continuum
Healthcare needs change throughout life. An unexpected illness may require attention today. A chronic condition may need regular review over many years. A new concern may call for specialist assessment.
We believe these should not be disconnected experiences.
To us, continuity means looking beyond the immediate consultation. It means helping patients understand their health, know what comes next, and receive appropriate follow-up or specialist input when needed.
It also means building relationships over time, understanding the person behind the symptoms and recognising that good care considers both today's concerns and tomorrow's needs.
That is the idea behind Continuum: care that connects one stage of life with the next, and one generation with another.
A personal note from our founder
For me, Continuum is personal.
My parents' work in family medicine and neurosurgery forms part of the foundation on which I am building this group. These are different fields of medicine, but both share the same responsibility: looking after people.
I founded Continuum to carry that foundation forward, to make healthcare more affordable, accessible and continuous, while preserving the personal attention that matters to patients and their families.
This photograph represents the family roots behind Continuum, and the connection between what came before and what we are building now.
My commitment is to honour that foundation through the way we care for our patients, support our teams and serve our communities.
