Who we are

This website is operated by Continuum Health Group ("CHG", "we") on behalf of its clinics: Continuum 24/7 Clinic Sengkang, Family Health Medical Centre, Chou Neuroscience Clinic, APEX Brain and Spine Surgical Centre. This policy explains how we handle personal data submitted through this website, in line with Singapore's Personal Data Protection Act 2012 (PDPA).

What we collect

When you book an appointment or send an enquiry through this website, we collect:

• your name, contact number and email address;
• your preferred clinic, date and time;
• any note you choose to add.

Please do not include medical details in website forms. Your doctor will discuss your health with you directly.

How we use it

• to arrange, confirm and manage your appointment;
• to reply to your enquiry;
• to contact you about your booking, for example if a time needs to change.

We do not use your website submissions for marketing without your separate consent, and we do not sell your personal data.

Who we share it with

• the clinic you choose, so it can handle your booking;
• service providers that help us run bookings and this website, such as our clinic management system, email provider and website host, who may only use your data to provide their service to us;
• Google, which provides our website analytics (see Cookies below);
• authorities, where we are required to by law.

Some clinics take bookings on their own booking pages, such as Plato or the Health Appointment System. Those pages are run by their providers under their own privacy terms.

How long we keep it

This website does not keep booking details in its own database. Your details are passed to the clinic you choose, which keeps them as part of its records for as long as needed for your care and to meet legal and medical record-keeping requirements.

Cookies

This website uses Google Analytics to count visits and see which pages are used, so we can improve the site. Google Analytics sets cookies and receives information about your visit, such as the pages you view, your approximate location and the type of device and browser you use. It does not receive what you type into our booking or enquiry forms.

You can block these cookies in your browser settings, or install Google's Analytics opt-out add-on. Apart from Google Analytics, this website uses only the cookies needed for it to work.

Keeping your data safe

We use reasonable security measures, including encrypted connections (HTTPS), restricted access to our systems and trusted service providers, to protect your personal data.

Your choices

You may ask to access or correct the personal data we hold about you, or withdraw your consent to our use of it. Withdrawing consent may mean we cannot arrange your appointment. To make a request, contact our Data Protection Officer at nurul@continuumhealthgroup.com.
