Dr Walter Chou
General PractitionerFounder, Continuum Health Group
Dr Walter Chou is a general practitioner and the founder of Continuum Health Group. He practises at Continuum 24/7 Clinic in Sengkang and Family Health Medical Centre in Ang Mo Kio, providing comprehensive primary and preventive medical care for adults and children.
His clinical interests include acute and after-hours medicine, family medicine, chronic disease management, cardiometabolic health, preventive medicine, diagnostic evaluation and the coordination of care across different stages of illness.
Before focusing on community medicine, Dr Chou gained clinical experience in both Neurology and Neurosurgery at the National Neuroscience Institute. This background continues to inform his approach to neurological symptoms, clinical deterioration and presentations that require timely investigation or specialist assessment, while his current practice remains centred on comprehensive general medicine.
Dr Chou believes that effective primary care should extend beyond treating the immediate complaint. His approach places emphasis on understanding the underlying clinical problem, identifying relevant risk factors, explaining treatment options clearly and ensuring that investigations, referrals and follow-up are carried through appropriately.
He founded Continuum Health Group around a simple principle: healthcare should be affordable, accessible and continuous across every stage of life.
Through the group, Dr Chou is developing a model of care that connects community primary care with diagnostics, preventive medicine and specialist services, allowing patients to move more smoothly from initial assessment to further treatment and ongoing follow-up.
His clinical philosophy combines evidence-based medicine, careful clinical judgement and continuity of care, with the aim of providing practical, comprehensive and well-coordinated healthcare for every patient.
