Our services

Chronic Disease Management

Ongoing management and monitoring of long-term medical conditions. Care may include medication review, blood investigations, monitoring and preventive risk management.

  • Diabetes
  • Hypertension
  • High cholesterol
  • Asthma
  • Other chronic medical conditions

Diabetes Care

Diabetes care aims to keep your blood sugar in a healthy range and lower the risk of problems with your heart, kidneys, eyes and feet. The doctor agrees personal goals with you, checks your HbA1c and other results, and reviews your medicines.

Yearly eye and foot checks are part of good diabetes care. Feet are examined at the clinic. Eye screening is arranged with an accredited retinal imaging provider, and we follow up the results with you.

  • HbA1c, kidney and cholesterol tests
  • Review and adjustment of tablets and injectable medicines
  • Help with blood sugar monitoring at home
  • Diabetic foot and nerve screening
  • Diabetic eye screening, arranged with an accredited provider
  • Referral to a specialist, podiatrist or dietitian when needed

High Blood Pressure & Cholesterol

High blood pressure and high cholesterol often cause no symptoms, but they raise the risk of heart attack, stroke and kidney disease. The doctor confirms the diagnosis, looks at your overall heart risk and agrees a plan with you.

Treatment may include changes to diet and activity and, where appropriate, medicines such as statins for cholesterol. Doses are reviewed at follow-up against the targets you agree with your doctor.

  • Blood pressure checks, with home readings where needed
  • Cholesterol and other blood tests
  • Heart disease and stroke risk assessment
  • Medicines where appropriate, reviewed for side effects
  • Regular follow-up and adjustment of treatment

Cholesterol medicine prices

For transparency, these are our prices for the original-brand cholesterol medicines we prescribe: statins, and ezetimibe on its own or combined with a statin. They are prescription-only medicines, prescribed only after a doctor's review, and your doctor decides which medicine and dose suit you.

Kidney, Thyroid & Gout Care

Diabetes and high blood pressure can affect the kidneys over time. Regular blood and urine tests track how well your kidneys are working, so treatment can be adjusted early. We work with kidney specialists when needed.

The doctor also assesses and follows up an underactive or overactive thyroid, with blood tests at the right intervals. For gout, treatment covers both flares and, where appropriate, long-term medicine to lower uric acid.

  • Kidney function and urine protein tests
  • Shared care with kidney specialists when needed
  • Thyroid blood tests and medicine adjustment
  • Treatment of gout flares
  • Uric acid monitoring and long-term gout medicine where appropriate
  • Referral to a specialist when needed

A hot, swollen joint

A sudden hot, swollen joint is not always gout. If you also have a fever or feel unwell, the doctor checks for a joint infection, which needs urgent hospital treatment.

Asthma & COPD Reviews

A regular review helps keep asthma and COPD under control between flare-ups. The doctor checks your symptoms, how often you need your reliever and how you use your inhalers, and adjusts your treatment.

You leave with a written action plan that tells you what to do if your breathing gets worse. Breathing tests (spirometry) can be arranged to confirm the diagnosis where needed.

  • Review of symptoms and flare-ups
  • Inhaler technique check
  • Adjustment of preventer and reliever treatment
  • Personal written action plan
  • Breathing tests (spirometry) arranged where needed
  • Flu and pneumococcal vaccination advice

Medication Review

Taking several medicines, from different doctors, can lead to doubled doses, side effects or medicines that do not work well together. The doctor goes through everything you take and checks that each medicine is still needed and safe.

Bring all your medicines in their packets, including supplements and anything bought over the counter. Any changes are explained to you and shared with your other doctors where needed.

  • Review of prescribed, over-the-counter and supplement use
  • Check for interactions and side effects
  • Stopping or reducing medicines that are no longer needed
  • A clear, updated medicine list
  • Changes shared with your other doctors where needed

Go to A&E or call 995 instead if
